Abstract Scope |
The incident involved a steam accumulator that was a component part of a steam generating facility. The accumulator ruptured during normal operating conditions. The rupture involved the complete fracture of a circumferential weld joining the bottom head to the shell. Metallurgical analysis was used to determine that the crack had been in place since the time the accumulator was first manufactured. A leak took place and a repair was attempted. The process to repair the leak failed to detect the condition of defect. The investigation determined that the repair was not conducted in accordance with the requirements of the National Boiler Inspection Code (NBIC). |
